Introduction
 We collect and use your personal information to provide oral health care. We collect information directly from our patients or persons who have authority to provide this information. As an example, we may collect personal contact information such as addressees and phone numbers, information for billing purposes and an up to date medical history. Our commitment to your privacy extends to meeting or exceeding the legislated requirements relating to personal information.
Accountability
We are responsible and accountable for all personal information in our possession and under our control. We have designated Tori Gray as our Privacy Officer. She endeavors to ensure compliance with our privacy policy by all staff members.
Identifying Purposes for Collecting Information
We collect and use personal information for the following purposes:
- to provide your oral health care and manage related issues;
- to obtain payment for your treatment including contacting third parties if necessary to obtain payment for these services;
- to maintain current patient information in our database;
- to establish and maintain communication with existing patients and patients referred to our office;
- to contact other health care practitioners for information with regard to your medical history in order to provide safe treatment;
- to support the business activities of this oral surgery clinic including, but not limited to, quality assessments, employee reviews, training and licensing.
If you do not wish to receive correspondence from our office, we will respect your wishes as long as our ability to collect payment for services rendered is not compromised in so doing.
Obtaining Your Consent
Your knowledge and consent to the collection, use and disclosure of personal information are important to us. We rely on the following to assess your consent:
- your voluntary compliance in providing your personal information directly to us; and,
- your expressed consent obtained via the Privacy Consent Form.
It is your right to withdraw your consent at any time. Our Privacy Officer will inform you of the resulting repercussions of withdrawing consent. In certain situations, withdrawing consent could result in our being unable to provide treatment.

Limiting Collection of Personal Information
We will only collect personal information necessary for the purposes previously mentioned. We will only collect personal information by fair and lawful means.
Limiting Use, Disclosure and Retention of Personal Information
 We do not sell personal information. We will not disclose your personal information to a third party not affiliated with us. At times, it is necessary for us to contact another physician who is familiar with your present medical status; this is done in an effort to provide safe treatment for you, the patient.
Your information is accessed by our employees on a strictly need-to-know basis.
Your personal information will be retained for as long as it is required in order for us to fulfill the purposes for which it was obtained. When your personal information is no longer required in our office, it will be destroyed in a safe and secure manner.
Accuracy
We endeavor to maintain accurate and current personal information for all of our patients so that we may fulfill the purposes for which your information is being held. If your personal information changes, we ask that you notify us, and once again provide consent for our retention of your information.
Protecting Your Personal Information
We take every reasonable precaution to protect your personal information in ways appropriate to its sensitivity and confidential nature. Your personal information that is recorded in your chart is kept under supervision by a trusted employee. If this is not possible, it is stored in a supervised filing cabinet or storage room. Every reasonable effort will be made to secure your personal information that is stored electronically in our computer system through the use of passwords and encryption measures.

We have prepared and posted this privacy policy in an effort to keep you, our patient, informed. Additional queries are welcome and will be addressed by our Privacy Officer.
