For your convenience, we accept cash, Visa, MasterCard and debit cards. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Our office makes every effort to remain compliant with the privacy policies that have been set forth by both the Federal (PIPEDA) and Provincial (PHIPA) Governments. We collect and use your personal information to provide health care. We collect this information from our patients or persons who have the authority to provide this information such as substitute decision makers. Our commitment to your privacy extends to meeting or exceeding the legislated requirements relating to personal information. In certain situations, we will need to contact other healthcare professionals to request or share your medical information so that we can provide care in our office. You will be advised if we feel that this step is necessary.
PHIPA provides a set of rules for the collection, use and disclosure of personal health information, and includes the following provisions:
